How It Works

A clear, four-step admissions process

From first contact to move-in, we keep the process transparent and the communication consistent.

Initial contact

Call, email, or submit the referral form below. We respond within one business day to discuss the prospective resident's needs and current availability.

Needs assessment

We review the referral, medical history, and care plan. Our team determines whether our home is the right fit for the individual's level of care.

Home visit

We invite the prospective resident and their family or case manager to tour the home, meet the care team, and ask questions in person.

Placement agreement

Once all parties agree, we complete the placement paperwork, coordinate with the funding source, and set a move-in date.

Who is a good fit for our home

We serve adults 18 and older whose care needs can be safely and appropriately met in a small, homelike residential setting.

  • Adults with intellectual or developmental disabilities (IDD)
  • Elderly adults who need personal care but prefer a home over a nursing facility
  • Adults with mental health diagnoses who benefit from structured, stable housing
  • Individuals transitioning from hospitals, group homes, or institutional settings
  • Adults whose care needs are compatible with a 3-bed AFC home environment

  • We serve adults with developmental disabilities, elderly and senior adults, and adults with mental health needs. We are licensed for three residents and accept individuals whose care needs match our staffing and home environment.

  • We work with Medicaid / MI Health Link, SSI/SSDI, MDHHS/DHS funding, and private pay. If you are unsure about funding eligibility, contact us — we can help navigate options.

  • Timelines vary depending on availability and the complexity of the individual's care needs. In straightforward cases with documentation ready, placement can happen within one to two weeks of initial contact.

  • Helpful documents include a current care plan or ISP, medical history, behavioral support plan (if applicable), and funding authorization. We can begin the conversation before all documents are in hand.

  • Absolutely. We encourage family involvement at every stage — from the initial tour through the transition and ongoing care. Families are partners in our care model, not bystanders.

  • We evaluate urgent placements on a case-by-case basis. If you have an emergency placement need, call us directly at (248) 388-5329 or (248) 252-3780 so we can respond as quickly as possible.

  • Our home has a maximum of three residents, which allows for a high staff-to-resident ratio and genuinely individualized attention. Staff are on-site 24 hours a day, every day of the year.
