We serve adults 18 and older, including seniors 55+, veterans, adults with physical or developmental disabilities, and individuals experiencing homelessness. Our home is licensed by the State of Michigan as an Adult Foster Care Home.

Common documents include a physician’s assessment or care plan, identification, insurance or benefit information (Medicaid, VA, SSI, etc.), and any relevant medical records. Our staff will guide you through exactly what’s needed for your situation.

We work to accommodate urgent placements and can often complete the intake process within 24–48 hours when a bed is available. Discharge planners and social workers are encouraged to call us directly for time-sensitive referrals.

Each day includes morning personal care assistance, home-cooked meals, medication administration, and a mix of rest, activities, and social time. Evening staff are on-site overnight for safety and support. We build routines around each resident’s preferences and needs.

Yes — we provide three home-cooked meals daily plus snacks. We accommodate dietary restrictions, allergies, and preferences. Communal dining is a cornerstone of our community, and we take nutrition seriously.

The Peace Garden is our outdoor wellness space designed for relaxation, reflection, and fresh air. We believe mental and emotional wellness are just as important as physical health, and the garden gives residents a calm, natural setting right at home.

Yes. We coordinate transportation to medical appointments, community programs, and other outings. We work with SMART, DDOT, and medical transport services to keep residents connected to the care and activities they need.

We accept Michigan Medicaid (including MI Health Link and Waiver programs), VA benefits, Social Security / SSI, private pay, and other approved funding sources. Our team can help you understand which benefits apply to your situation.

We can help connect you with the right resources. Our team works with the Michigan Department of Health & Human Services, the Area Agency on Aging 1-B, and veterans services organizations to help residents and families navigate benefits.

Absolutely. Family involvement is encouraged and welcomed. We ask that visits be coordinated in advance so we can ensure the best experience for your loved one and other residents. Contact us to arrange a visit.

Our staff communicates proactively with families about health changes, care updates, and any concerns. We believe families are partners in care, and we maintain open lines of communication at all times.

We regularly reassess each resident’s care plan and adjust services as needs evolve. If a resident’s needs exceed what we can safely provide, we will work with the family and appropriate agencies to find the right next step.

A basic referral should include the individual’s name, age, primary diagnosis or care needs, current location (hospital, shelter, etc.), and funding source if known. We can work with incomplete information and help gather what’s missing.

Yes. Lilly’s Loving Care LLC is a Michigan-licensed Adult Foster Care Home, fully compliant with all state regulations. We welcome questions from discharge planners and can provide licensing documentation upon request.
